How to Apply
Eligibility – Basic eligibility requirements are as follows:
- Child is 21 years of age and under, or expecting mothers travelling for their baby
- Permanent residence is within the Thunder Bay District
- Required to travel to a medical centre outside the city of Thunder Bay
General Requirements
- All applications require supporting documents
- Incomplete application will be returned unprocessed
- Submissions of an application authorizes the Friends of Olivia Foundation to cross reference information provided
- Please allow 4-6 weeks for your application to be processed
- Special consideration will be giving to situations that are time sensitive
